Fetter Health Care Network receives official Patient-Centered Medical Home recognition

Fetter Health Care Network has officially received recognition as a Patient-Centered Medical Home (PCMH). Fetter is a Federally Qualified Health Center with over 20 locations throughout the Lowcountry.

Fetter Health Care Network received recognition by demonstrating satisfactory performance in:

  • having team-based care and practice,
  • knowing and managing patients,
  • having patient-centered access and continuity,
  • practicing care management and support,
  • ensuring care coordination and care transitions, and
  • consistently measuring performance and improving quality

“When you choose a Patient-Centered Medical Home, you can count on receiving comprehensive care services that are fully focused on the patient from a strong team of providers,” said Aretha R. NCQA’s PCMH program is the most widely-adopted evaluation program in the country, with approximately 13,000 practices recognized.
