Fetter Health Care Network receives official Patient-Centered Medical Home recognition

Fetter Health Care Network has officially received recognition as a Patient-Centered Medical Home (PCMH). Fetter is a Federally Qualified Health Center with over 20 locations throughout the Lowcountry.
Fetter Health Care Network received recognition by demonstrating satisfactory performance in:
- having team-based care and practice,
- knowing and managing patients,
- having patient-centered access and continuity,
- practicing care management and support,
- ensuring care coordination and care transitions, and
- consistently measuring performance and improving quality

NCQA’s PCMH program is the most widely-adopted evaluation program in the country, with approximately 13,000 practices recognized.
