Bon Secours St. Francis Health System tied for first in the nation for lowest 30-day heart failure readmission rate

GREENVILLE, SC – Bon Secours St. Francis Heath System was recently recognized by Becker’s Hospital Review for being top in the nation with the lowest 30-day heart failure readmission rate.

According to data from Hospital Compare, St. Francis Downtown has an average readmission rate of 16.6 percent, which is significantly lower than the national average of 22.7 percent.  St. Francis Downtown was tied for first on the list of the top 53 hospitals in the nation with the lowest heart failure readmission rates.

The award-winning Cardiac Care program at Bon Secours St. Francis is among the top 10 percent in the nation according to Healthgrades and includes a Cardiac Care Unit with the latest technology and a 24/7 Chest Pain Center.  Bon Secours St. Francis maintains a door-to-balloon time (the time measured from when the patient enters the emergency room to when the blockage is cleared) that is well below the national average.

“We are proud to have one of the lowest 30-day heart failure readmission rates in the nation,” said Saria Sacoccio, MD, chief medical officer/Bon Secours St. Francis Health System.  “We are constantly working to improve our processes to reduce heart failure readmissions.  At Bon Secours St. Francis, we have a Heart Care Navigator who works closely with patients and their caregivers to ensure patients receive the care they need once they leave the hospital.  We also have established a pharmacy home health program so that patients recovering at home have access to a pharmacist.  Ensuring patients are taking their medications correctly once they leave the hospital is just one of the many ways we are working to reduce readmission rates.”

“We have also developed risk calculators that help determine how likely it is that a patient will be readmitted,” said Dr. Saccocio.  “These risk calculators help physicians determine the appropriate model of care based on the patient’s calculated risk for readmission.  We are proud of the care we deliver to our heart patients and the systems we have put in place to lower heart readmission rates and we will continue to strive to decrease these rates even further.”

The Cardiac Care program at Bon Secours St. Francis has received multiple recognitions and awards.  In 2014, Healthgrades awarded Bon Secours St. Francis the Cardiac Care Excellence award and named Bon Secours St. Francis one of America’s 100 Best Hospitals for Cardiac Care.  Also in 2014, Modern Healthcaremagazine named St. Francis downtown fourth in the nation for best 30-day readmission rate and the hospital was recognized as a Top Performer by The Joint Commission in heart attack, heart failure and pneumonia.
